Historical Entries w/ Reason Codes - Employee Add or Edit a Punch Entry

Description: In this topic, the employee will learn how to enter a punch entry and edit a punch entry in the web portal after the shift has been completed (historical entry).

Enter a Punch Entry - Web Portal (Historical Entry)

1. Log in to the DCI Web Portal
   a. All states except WI/NC Cap: acumen.dcisoftware.com
   b. WI/NC Cap Only: outreach.dcisoftware.com

2. Click the blue Add Entry button on the dashboard

3. Complete the Add New Entry form wizard

   1. Entry Type - Punch is the Default. Other options may be available.

   2. Employee Name - Auto-populates the name of the employee logged in

   3. Account Type - Hourly will auto-populate

   4. Client - Type and select the client name from list. Type a minimum of three characters to generate results and select the Client's name from the list

   5. Service Code - The service code(s) authorized for the client. Select the one that is most appropriate for the service rendered.

   6. Service Date - The date the service took place. 

   7. Check In/Check Out - Enter the clock in/clock out times. Both fields are required. Clicking in the field displays a time drop-down in half hour increments. Specific time can be entered directly into the field.

   8. EVV Method - For web portal time punches, the option will be Portal Signoff. The only exception is if FOB is enabled for the organization.

   9. Clock in EVV Location - Where the service was performed. There may be one or more options available depending on the client. This field will not be visible if EVV locations have not been defined for the client. This field will not be visible if EVV locations have not been defined for the client. 

   10. Clock out EVV Location - See above. This should remain the same as the Clock in location unless the service ended in a different location. There may be one or more options available depending on the client. This field will not be visible if EVV locations have not been defined for the client. 

   11. Check Out Date - The checkout date will be automatically generated based on the check in day and cannot be edited

   12. Add Reason Codes - may require a reason code when entering a punch, explaining why a non-EVV-compliant method of time entry has been added. Select a reason code from the Add Reason Codes drop-down. 

   13. Add Reason Code Note - Some Reason Codes require an additional field called Add Reason Code Note to be completed. Enter a free form note to support the chosen reason code(s). 

   14. Notes (optional) - Enter notes 

   15. Attachment (optional) - Click the blue Add Attachment hyperlink to add an attachment. Examples include receipts, photos, etc.

   16. Statements/Tasks - Visible only if required by your program. If required, the entry cannot be saved without selecting one or more.

4. Click Save and Yes to confirm

Edit a Punch Entry - Web Portal (Historical Entry)

1. 1. Log in to the DCI Web Portal
      a. All states except WI/NC Cap: acumen.dcisoftware.com
      b. WI/NC Cap Only: outreach.dcisoftware.com

2. Click Home on the main menu

3. Click Entries on the submenu

4. Search for the entry by using the search filters and click Search

5. Click on the entry in the table

6. Click Actions

7. Click Edit Entry in the drop-down menu

8. Edit fields as needed in the Edit Entry form wizard

9. Click Save and Yes to confirm

10. The original punch status changes to Rejected

11. A new punch with the updates is generated and is in Pending status

    1. The new punch will have a Ref. Entry # listed which links it to the original punch

12. To view the original and new punch, select Entries on the submenu.

13. Both punches are listed in the Entries table below the search box
